【活動事項 Workshop Task】

• 參與者於當日（4/20）須配合講師課程指示並完成線上問卷調查填寫事宜。
  All participants must follow the instructions given by the instructor and complete the online questionnaire on the same day(4/20).
• 經講師及工作人員確認後，始得領取研習證書。
  The certificate will be issued after the questionnaire has been checked by the instructor and staff.
  
   

【注意事項 Note】

• 為珍惜活動資源，經自行評估可全程參與方可報名。
  The workshop is only open to the participants who make full commitment to the program.
• 於系統完成報名並非正式錄取，請以理學院網站公告之錄取名單為準。
  Completing the registration in the system does not mean that you are admitted, please check the admission list announced on the website of the College of Science.
• 倘若報名人數過多，將依情形停止報名及公告錄取名單。
  如遇有正取者放棄之情形，本院將以報名時所填之聯絡方式依序聯繫備取者。
  備取者須接到通知後才算正式錄取，未接到通知請勿出席活動。
  Substitute name list will be announced.
  Please do not show up unless you have received confirmation calls.
• 因故臨時無法參與活動，敬請於七日前來信或來電取消報名。
  Please inform the College of Science by phone call before 7 days, if you could not attend the workshop for any reasons.
• 倘若參與者活動當日已排定必選修課程，
  敬請尊重師長，取得授課教師同意並提供相關證明文件（如：同意信件擷圖）。
  For those who have classes on the same day as the workshop , please make sure that you have informed your professors, obtained their permission for you to be excused from class to attend this workshop , and provided relevant supporting documents (e.g. screenshot of consent letter).

【溫馨提醒 Friendly Reminder】

• 本活動得採計築夢展翼翻轉計畫時數。
  This activity can be counted towards " 築夢展翼翻轉計畫 " hours.
• 本活動之築夢展翼翻轉計畫學習護照時數核章程序，請先自行撰寫內容（活動名稱請參考下方錄取名單場次名稱，時數為15小時<4/18 13：30～16：30；4/19 09：00～12：00、13：30～16：30；4/20 09：00～12：00、13：30～16：30>），再於活動當天（4/20）攜帶學習護照至理學院辦公室（屏師校區敬業樓2樓）辦理。
  For this activity, please write down the content of the study passport (please refer to the following list for the name of the activity, the duration is 15 hours<4/18 13：30～16：30；4/19 09：00～12：00、13：00～16：30；4/20 09：00～12：00、13：00～16：30>) and bring the study passport to the office of the College of Science (2F, Jing Ye Building, PingShi Campus) to apply for the study passport on the day(4/20) of the activity.
